PARENT/GUARDIAN'S CONSENT:
By completing this registration, I hereby consent that I will not hold the VRC, its Directors, Coaches, and members liable or responsible for any injury or accident that might happen to my child (named above) while attending the VRC Badminton Training Program being hosted by the Club.
HEALTH DECLARATION:
By completing this registration, I declare that I will not allow my child to attend ANY Badminton Programs if he/she has the following:
1) He/she has travelled outside of Canada or had close contact with anyone that has travelled outside of Canada in the past 5 days.
2) He/she has had close contact with anyone with a confirmed or probable case of COVID-19.
3) He/she has any COVID-19 symptoms, such as new or worsening cough, shortness of breath, sore throat, runny nose, sneezing or nasal congestion, hoarse voice, difficulty swallowing, new smell or taste disorder(s), nausea/vomiting, diarrhea, abdominal pain, unexplained fatigue/malaise, chills, fever, headache, or new muscle ache.
